not sell your protected health information.
HIS does not provide your e-mail address or other contact
information to outside mailing lists or mass marketing programs.
HIS does not sell your information, nor
disclose your private information, for any reason except as described

Health Insurance Specialists, Inc. (HIS) does not disclose protected
health information about our clients to anyone, except as required by
law, or as is necessary to serve your needs in the normal business
practices of an insurance agency.
During the normal course of business, and
as a part of our efforts to serve your insurance needs, HIS may share
the personal information described below with companies or persons
affiliated with HIS, such as an Insurance Company, or others with which
we have contracts to perform functions on our behalf. If we are
unable to meet your insurance needs, we may provide information to
another agent or entity who may perform these professional services on
your behalf. All of these
persons or companies, which act with us on our behalf, are contractually
obligated to keep the information that we provide to them confidential
and to use the information only to provide the services we have asked
them to perform for you and us.
Former Customers and Prospective
Customers
We will continue to safeguard your information as described in this
notice, without regard to whether you are a current client, a former
client, or a prospective client.
Security Information
HIS maintains policies and practices that protect the security and
confidentiality of customer information. This includes: limiting
non-public protected health information to employees who need the information in
order to perform their job duties, shredding purged documents,
maintaining user passwords, and protecting information through
security-enhancing software such as intrusion detection software.

As a part of providing you with represented products and services, we
may collect public and non-public personal information, about you and/or
your dependents, including:

applications, quote requests, or other documents, such as: name,
address, telephone number, e-mail addresses, social security number,
date of birth, gender, marital status, assets, income, personal
health history, height, weight and claims history.

account number, account balance, policy coverage, and payment and
claims history.
